Abstract
35 of 47 ankle joint endoprostheses implanted between 1977 and 1984 due to joint destructions caused by rheumatic diseases, were followed up.
The most impressive result - as is usual with endoprosthetic applications - was represented by alleviation of pain. With increasing observation period, x-ray film revealed a distinct absorption margin especially in the range of the tibia component, mostly without any clinical signs of loosening. Two joints are now due for repeat surgery because they are suspected of loosening.
Mobility was controlled both clinically and by x-ray film and remained below expectations with a value of very nearly 20° (+ 5° in the intertarsal region). Compared with the early postoperative findings this represents a marked reduction. The possible reasons for this reduction are discussed.
On the whole, the patients are much more content and satisfied with the results than one would expect on the basis of the objective findings.
Zusammenfassung
Nachuntersucht wurden 35 von 47 Sprunggelenksendoprothesen, die zwischen 1977 und 1984 aufgrund rheumatischer Gelenkdestruktionen implantiert wurden. Eindrucksvollstes Ergebnis war - wie üblicherweise bei endoprothetischen Versorgungen - die Schmerzreduzierung. Mit zunehmender Beobachtungsdauer zeigte sich bei gut der Hälfte der Patienten insbesondere im Bereich der Tibiakomponente röntgenologisch ein deutlicher Resorptionssaum, mehrheitlich ohne klinische Lockerungszeichen. Zwei Gelenke stehen wegen Lockerungsverdacht zur Reoperation an.
Die Beweglichkeit wurde klinisch und röntgenologisch kontrolliert und blieb mit knapp 20 Grad hinter den Erwartungen zurück (+ 5 Grad im Intertarsalbereich). Gegenüber den frühen postoperativen Befunden bedeutet dies einen deutlichen Rückgang, dessen mögliche Gründe diskutiert werden.
Insgesamt sind die Patienten deutlich zufriedener, als nach den objektiven Befunden zu erwarten wäre.
