Rheumaorthopädie heute - Operative Möglichkeiten bei der Behandlung der chronischen Polyarthritis der Schulter

Abstract

Rheumatic disease of the shoulder produces in the course of the disease destructive changes in the joint and the shoulder muscles. Hence, surgery of the shoulder should be effected early while preserving the muscular arthrogenous balance. Such surgery appears justified in view of the good results obtained after early synovectomy and also after late synovectomy. In reconstructive surgery, resecting interpositional arthroplasty has proved successful, since total endoprosthetic care of the shoulder does not yield good long-term results due to the loosening of the glenoid component. Physiotherapeutic aftercare directly after the operation and during follow-up is an essential factor in the treatment of the rheumatic shoulder syndrome.

Zusammenfassung

Der rheumatische Befall der Schulter führt im Laufe der Erkrankung zu destruktiven Veränderungen im Gelenk und an dem Muskelmantel der Schulter. Operative Eingriffe an der Schulter sollten daher frühzeitig bei Erhalt des muskulären-arthrogenen Gleichgewichtes durchgeführt werden. Die guten Ergebnisse nach Frühsynovektomien und auch nach Spätsynovektomien rechtfertigen diese Eingriffe. Bei rekonstruktiven Eingriffen hat sich die Resektions-Interpositionsarthroplastik bewährt, da die totalendoprothetische Versorgung der Schulter durch die Lockerung der Pfannenkomponente keine guten Langzeitergebnisse erbringt. Ein wichtiger Faktor in der Behandlung der rheumatischen Schulter ist die krankengymnastische Nachbehandlung direkt postoperativ und im weiteren Verlauf.
