Zusammenfassung
Bei 108 Patienten mit operativ (n = 63) bzw. klinisch (n = 45) gesicherter akuter Pankreatitis wurden das computertomographische und pathoanatomische Staging korreliert. Die computertomographische Differenzierung zwischen interstitieller und nekrotisierender Pankreatitis wies eine Sensitivität von 0,87 und eine Spezifität von 1,0 auf. Darüber hinaus war bei der Mehrzahl der Patienten eine exakte Größenbestimmung der intrapankreatischen Nekrosen durch Computertomographie möglich. Die hohe Treffsicherheit macht die Computortomographin zum Verfahren der ersten Wahl zu Frühdiagnose und Frühstaging der akuten Pankreatitis.
Summary
In 108 patients with acute pancreatitis, the CT findings were correlated with pathological staging; in 63 the diagnosis had been confirmed at operation and in 45 it was made on clinical grounds. CT differentiation between interstitial and necrotizing pancreatitis had a sensitivity of 0.87 and specificity of 1.0. In addition, exact estimation of the extent of intrapancreatic necrosis was possible. This high accuracy makes CT the methode of choice for the early diagnosis and staging of acute pancreatitis.
