Zusammenfassung
Mit dem paramagnetischen Kontrastmittel Gadolinium-DTPA hat sich die Diagnostik spinaler Raumforderungen bezüglich der Ausdehnung von Tumoren und der differentialdiagnostischen Zusatzinformationen im Vergleich zur Nativ-Kernspintomographie verbessert. Eine wichtige Indikation stellt die Verwendung von Gadolinium-DTPA bei der Abgrenzung intramedullärer Tumoren dar sowie die Differenzierung von Syrinx und zystischem Tumor. Auch bei extramedullären Raumforderungen ist eine bessere Abgrenzung der Raumforderung, vor allem bei Tumoren mit hoher Kontrastmittelaufnahme wie Neurinomen und Meningiomen sowie Chordomen und Lymphomen, möglich. In der Differenzierung zwischen den wenig KM-aufnehmenden Narben und den meist deutlichen KM-aufnehmenden Rezidivtumoren erbringt die Kernspintomographie mit Gadolinium-DTPA meist die entscheidende Information.
Summary
In spinal lesions GD-DTPA has led to a better diagnosis of tumors concerning the extent and differential diagnosis. Important indications for the application of GD-DTPA are intramedullary tumors: excellent information with respect to extent and differentiation cyst from tumor. At extramedullary intradural tumors we got distinction of extramedullary from intramedullary tumors and more information in the differential diagnosis for example in cases of neurinomas and meningiomas. Additional information is also obtained by means of GD-DTPA in extradural tumors: better delineation from the myelon, better possibilities to differentiate between meningiomas, neurinomas and scar and tumor.
