Synovektomie des Handgelenkes

Abstract

The wrist joint is involved in a great majority of rheumatoid arthritis patients. Surgery is indicated if conservative therapy remains unsuccessful. Synovectomy of the wrist joint is a preventive measure direct at preserving the joint. It is usually performed as a combined operation together with tenosynovectomy and a resection of the head of the ulna. Surgery has now mostly been standardized. The incision is of the straight type, over the extensor side of the wrist, giving the surgeon a clear view. After the joint has been opened, thorough synovectomy of the radiocarpal joint must be effected. The necessary combined surgery manipulations are then performed. Today wrist joint synovectomy is an established procedure in the surgical treatment of the hand of the rheumatoid arthritis patient. Follow-up examinations by the author have confirmed a positive influence particularly on pain and swelling. Functional ability increases, a fact which is explained by the reduction of pain. The operation prevents progressive destruction of the wrist.

Zusammenfassung

In der überwiegenden Zahl der Fälle bei chronischer Polyarthritis ist das Handgelenk betroffen. Bei erfolgloser konservativer Therapie sind operative Maßnahmen angezeigt. Die Synovektomie des Handgelenkes ist im Sinne der Gelenkerhaltung ein präventiver Eingriff. Sie wird als Kombinationseingriff meist mit einer Tenosynovektomie und einer Ulnaköpfchenresektion durchgeführt. Das operative Vorgehen ist heute weitgehend vereinheitlicht. Eine gerade Schnittführung über der Streckseite des Handgelenkes gestattet eine gute Übersicht. Nach Eröffnung des Handgelenkes ist eine gründliche Synovektomie des Radio-carpal-Gelenkes vorzunehmen. Die notwendigen Kombinationseingriffe schließen sich an. Die Handgelenkssynovektomie hat heute einen festen Platz in der operativen Behandlung der Hand des Polyarthritikers. Eigene Nachuntersuchungen ergeben vor allen Dingen einen positiven Einfluß des Eingriffs auf Schmerzen und Schwellung. Der Gewinn an Funktion nach dem Eingriff wird durch die Schmerzreduzierung erklärt. Zunehmende Destruktionen des Handgelenkes werden durch diesen Eingriff vermieden.
