Beitrag zum multiplen endoprothetischen Gelenkersatz an Hüft- und Kniegelenken

Zusammenfassung

Es wird über den multiplen Gelenkersatz an 38 Hüftund 42 Kniegelenken bei 23 extrem behinderten Patienten berichtet. Die verschiedenen anästhesiologischen und operativen Probleme werden aufgezeigt. Der Operateur, der sich an den multiplen alloarthroplästischen Gelenkersatz heranwagt, sollte über die nötige klinische und auch operative Erfahrung in der Endoprothetik der Hüft- und Kniegelenke, insbesondere bei Rheumatikern, verfügen. Der Einsatz lohnt sich, aber die Risiken darf man nicht unterschätzen. Rheumatiker sind immer kooperativ eingestellt. Sie nehmen auch das Risiko des multiplen Gelenkersatzes in Kauf, wenn man sie davon überzeugen kann, daß ihre ohnedies schon große Hilflosigkeit durch die Operationen, auch bei ungünstigem Ausgang nicht noch schlimmer wird.

Abstract

The article reports on multiple arthroplasty in 38 hip and 42 knee joints in 23 highly disabled patients. The various problems of anaesthesiology and surgery are explained. The surgeon who undertakes to perform multiple joint alloarthroplasty, should command over the requisite clinical and surgical experience in endoprostheses of the hip and knee joints, especially in rheumatics. Although this is a very rewarding task, the risks involved should not be underestimated. Rheumatics are always co-operative by nature. They are prepared to accept the risk of multiple joint replacement if they can be convinced that their disability, which is considerable as it is, will not become worse by surgery, even if the Operation is unsuccessful.
