Zusammenfassung
Die Befunde von 308 Kieferarthrogrammen (280 Patienten) mit selektiver Kontrastmittelinjektion in den unteren Gelenksraum werden beschrieben. Ein nach vorne luxierter Meniskus wurde in 231 Fällen (75 %) diagnostiziert. In 99 Fällen (32 %) reponierte sich der luxierte Meniskus jeweils spontan beim Öffnen des Mundes, während er in 132 Fällen (43 %) permanent nach vorne luxiert war. Die arthrographische Diagnose eines luxierten Meniskus wurde in 108 Patienten, bei denen eine operative Behandlung durchgeführt worden war, bestätigt. In 18 Patienten (6 %) mit Meniskusluxation ohne spontane Reduktion wurde zusätzlich eine Perforation des Meniskus oder seines Bandapparates arthrographisch diagnostiziert, die aber in 2 Fällen operativ nicht bestätigt werden konnte. Die Bedeutung der frühzeitigen Diagnose einer Meniskusluxation für eine optimale Therapie wird diskutiert.
Summary
The findings in 308 TMJ arthrograms (280 patients) with selective contrast injection into the inferior compartment are described. An anteriorly luxated meniscus was diagnosed in 231 cases (75 %). In 99 cases (32 %) the meniscus returned to a normal position spontaneously during opening of the mouth, whereas in 132 cases (43 %) the subluxation was permanent. In 108 patients (6 %) with irreducible meniscus luxation, perforation of the meniscus or of the capsule was diagnosed, but in two cases this could not be confirmed at operation. The importance of early diagnosis for optimal treatment is discussed.
