Zusammenfassung

Bei der 16jährigen Patientin traten, nachdem sie bei einem Skiunfall mit dem rechten Auge auf die Skispitze gefallen war, Kopfschmerzen, Monokelhämatom und Lidptose auf. Nach Abklingen des Hämatoms fanden sich keinerlei äußere Verletzungen, während sich im CT eine Blow-out-Fraktur des Orbitadaches und -bodens zeigte. Da keine Augenmuskulatur betroffen war, stellte sich aus ophthalmologischer Sicht keine Indikation zum operativen Eingriff. Auch eine erwartete Rhinoliquorrhö aufgrund der Verbindung zwischen Frontalhirn und Kieferhöhle stellte sich nicht ein, so daß auch die neurochirurgische frontobasale Revision nicht indiziert war und die Patientin entlassen werden konnte.

Abstract

Due to a ski accident a 16-year old girl suffered from headache, periorbital hematoma and ptosis of the right eyelid. No penetrating injury could be seen, but in the CT scan a blow-out fracture of the orbital roof and floor was verified. Since there was no evident lesion of the eye muscles, an operative indication could only be a neurosurgical one. Because of the traumatic connection between frontal cerebrum and sinus maxillaris we expected a CSF rhinorrhoea, which could not be confirmed.
