Spontanheilung des Astigmatismus nach 7 mm Korneoskleralschnitt zur Implantation einer Hinterkammerlinse

Zusammenfassung

Der kurze Korneoskleralschnitt, senkrecht in ⅓ Wandtiefe vorgeritzt und schräg ins Hornhautgewebe vollendet, bedarf nach unserer Erfahrung zum Wundverschluß nur zweier, ⅓ Wandstärke fassende 9-0 Nylon-Einzelknopf- oder X-Nähte in Kombination mit 1-3 Seidennähten. Der nach 2 Wochen wechselnd hohe postoperative Astigmatismus ist innerhalb von 3 Monaten spontan soweit gebessert, dass wir eine intraoperative Keratometrie für unnötig halten.

Summary

The 7 mm corneoscleral opening to implant a posterior chamber lens after phakoemulsification is performed in 2 separate steps: a vertical incision with a round conical blade (Beaver 54) to one-third of the scleral thickness, the final penetration with the razor blade and scissiors sloping forward into the cornea. Satisfactory wound closure is achieved with 2 9-0 nylon single-knot sutures combined with 3 8-0 silk sutures, all of which are done with an Alcon CU-1-⅜ round needle. Most postoperative astigmatism decreases spontaneously within 3 months to such a degree that there is no need for keratometry during surgery.
