Silikonölimplantation. Erste Erfahrungen mit einer modifizierten Technik

Zusammenfassung

Nach anfänglichen Mißerfolgen in der Behandlung der massiven periretinalen Proliferation (MPP) im Stadium III bis IV - Silikonöl gelangte dreimal retroretinal und wurde wieder abgesaugt - haben wir in 5 Fällen nach zuvor erfolgter Luftfüllung des Glaskörperraumes bei bereits expandierter Netzhaut Silikonöl implantiert. Bei einer Beobachtungszeit zwischen 10 Wochen und 6 Monaten sind bisher keine Komplikationen aufgetreten. 2 Fälle zeigen weiterhin Schrumpfungsfalten, so dass an eine spätere Entfernung des Silkons nicht gedacht werden kann. Bei 3 Fällen ist die Netzhaut jedoch völlig angelegt, die Rezidivgefahr nach panretinaler Fotokoagulation gemindert, so dass zur Vermeidung von Spätkomplikationen das Silikonöl nur temporär im Glaskörperraum verbleiben soll.

Summary

Following initial failures in the treatment of massive periretinal proliferation (MPP) stages III to IV - in 3 cases silicone oil penetrated behind the retina and had to be removed immediately - the implantation technique was modified and was successful in 5 cases after expanding the retina with air before implanting the silicone oil. In follow-up periods ranging from 10 weeks to 6 months no complications occurred. In 2 cases traction folds prohibit a removal of the silicone oil. However, in 3 cases the retina is completely attached and after panretinal photocoagulation the danger of a recurrence of the detachment has been diminished; it is therefore planned to remove the silicone oil from the vitreous cavity to avoid late complications.
