CT-Diagnostik bei Harnleiterobstruktion

Zusammenfassung

Es wird über eine retrospektive Auswertung von 28 CT-Untersuchungen bei Harnleiterobstruktionen berichtet. Bei 27 Patienten konnte eine Lokalisation des obstruktiven Prozesses anhand der Computertomographie erfolgen, in 25 Fällen zusätzlich eine Aussage zur Art der Obstruktion. Hierbei wurde zwischen Kompression von außen, intrakanalikulärer Obstruktion und Ureterwandstriktur unterschieden. Bei 19 Patienten gelang unter Einbeziehung anamnestischer Daten eine Aussage zur Ätiologie. Anatomische und pathologische Aspekte der Harnleiter werden knapp skizziert. Der pathophysiologische Mechanismus bei Harnleiterobstruktion wird anhand der Literatur diskutiert. Auf die Schwierigkeiten bei der CT-Diagnostik von Harnleiterobstruktionen wird eingegangen.

Summary

A retrospective study of 28 CT examinations of obstructed ureters is reported. In 27 patients it was possible to localise the obstructive process by CT; in 25 cases it was possible to determine the cause of the obstruction. It was possible to distinguish between compression from the outside, obstructive causes in the ureter and strictures of the ureteric wall. In 19 patients it was possible, by taking into account the history also, to determine the aetiology. Anatomical and pathological aspects of the ureter are briefly outlined and the mechanism of ureteric obstruction is discussed in relation to the relevant literature. Difficulties in the CT diagnosis of ureteric obstruction are included.
