Computertomographie bei akuter Pyelonephritis

Zusammenfassung

Bei 30 Patienten mit akutem renalen Infekt wurde eine Computertomographie der Nieren durchgeführt (akute suppurative Pyelonephritis, akuter renaler Abszeß, infizierte Zyste, Pyonephrose, Steinperforation, retroperitonealer Abszeß). Die Computertomographie liefert im Vergleich zu Urogramm und Sonographie genauere Information über das Ausmaß der entzündlichen renalen und extrarenalen Veränderungen. Dadurch wird die Wahl des therapeutischen Vorgehens hinsichtlich medikamentöser Behandlung oder chirurgischer Intervention entscheidend beeinflußt. Angiographie und retrograde Pyelographie kommen gezielt zur Anwendung, insbesondere bei Verdacht auf akute bakterielle Nephritis oder Nierenvenenthrombose bzw. zum Nachweis einer Ureterobstruktion.

Summary

Computer tomography of the kidneys was performed on 30 patients with acute renal infections (acute suppurative pyelonephritis, acute renal abscess, infected cyst, pyelo-nephrosis, calculus perforation, retroperitoneal abscess). Computer tomography provided more accurate information concerning the extent of the renal and extra-renal inflammatory process than did the urogram or sonogram. This may significantly affect the choice of treatment, particularly concerning the use of drugs or of surgery. Angiography and retrograde pyelography may be used in selected cases, especially where there is a suspicion of acute bacterial nephritis, renal vein thrombosis or ureteric obstruction.
