Zusammenfassung
Die Einführung einer Anzahl alternativer, nicht-invasiver Therapiekonzepte der BPH-Therapie macht eine Quantifizierung der Obstruktion bei BPH not-wendig. Die Reproduzierbarkeit von Druckflußmessungen zur Quantifizierung der Obstruktion ist umstritten.
Bei 128 Patienten wurde eine Urodynamische Messung mit freiem Flow, und computerunterstützter Druckflußmessung mit dem Programm AUDIT™ durchgeführt. Eine dritte Messung erfolgte bei 25 Patienten. Die mechanische Obstruktion wurde nach CHESS klassifiziert. Die Reproduzierbarkeit wurde durch den t-Test und Wilcoxon-Test berechnet.
Alle urodynamischen Parameter waren statistisch reproduzierbar. Die CHESS-Klassifikation war ebenfalls statistisch reproduzierbar.
Druckflußmessungen stellen eine statistisch reproduzierbare Methode zur Quantifizierung der mechanischen Obstruktion bei BPH dar. Zum Ausschluß artifizieller Artefakte sollten Mehrfachmessungen durchgeführt werden.
Abstract
The use of an increasing number of alternative treatment modalities in BPH has raised the importance of urodynamic quantification of BPH-obstruction. There is some doubt as to the problem concerning the general reproducibility of pressure flow analyses as the main tool.
128 patients underwent two routine urodynamic investigations with free flow and computerised pressure-flow measurement with the program AUDIT™. A third investigation was performed on 25 patients. The mechanical BPH-obstruction was classified according to CHESS. The reproducibility was tested with paired t-test and Wilcox-on.
All urodynamic parameters were fully statistically reproducible. The test- re-test reliability of CHESS classification was found to be sufficient.
Pressure flow analysis is a statistically reproducible methode to quantify subvesical obstruction. A repeated voiding should be investigated in urodynamics to eliminate possible artificial artefacts and to increase the diagnostic accuracy of measurements.
