Welche Therapie bei Lipomatosis pelvis?

Zusammenfassung

Die Lipomatosis pelvis ist eine benigne Erkrankung, die durch eine tumorimitierende Fettneubildung im kleinen Becken gekennzeichnet ist. Durch Kompression der Harnwege entsteht eine Harnstauung, aus der schwerwiegende urologische Komplikationen bis hin zur Urämie resultieren können. In 75-80 % tritt gleichzeitig eine Cystitis glandularis auf, die als Präkanzerose des Adenokarzinoms aufzufassen ist. Es wird über einen 50jährigen Patienten berichtet, bei dem wegen Lipomatosis pelvis eine radikale Cystoprostatovesikulektomie mit kontinenter Harnableitung durchgeführt wurde. Anhand einer Literaturrecherche werden Symptomatik, Diagnostik und Therapie dieser Erkrankung diskutiert.

Abstract

Lipomatosis pelvis is a disease characterized by tumorlike overgrowth of normal fat in the pelvis. Due to uninary tract obstruction severe urological complications up to uraemia can arise. In 75-80 % a cystitis glandularis is observed at the same time which has to be regarded as premalignant for adenocarinoma of the bladder. We describe a 50-year old male patient with lipomatosis pelvis where we performed a radical cystoprostatovesiculectomie and a continent urinary diversion. Reviewing the literature the symptoms, diagnostic procedures and therapy of this disease are discussed.
