Zusammenfassung
Die antegrade skrotale Sklerotherapie wurde bei 40 Jungen zwischen 10 und 15 Jahren
zur ambulanten Behandlung der Varicocele testis eingesetzt. 36 Patienten hatten linksseitige,
4 weitere beidseitige Varikozelen. Es lagen 18 drittgradige (40,9 %), 22 zweitgradige
(50 %) und 4 erstgradige Varikozelen (9,1 %) vor. Von 44 Eingriffen wurden 29 in Lokalanästhesie,
15 in Periduralanästhesie vorgenommen. Bei der Kontrolle nach 1-27 Monaten (x = 9,1
Monate) fand sich eine persistierende Varikozele (III°) (Persistenzrate 2,3 %), die
erfolgreich in zweiter Sitzung antegrad verödet wurde. Als Komplikation zeigte sich
ein Wundhämatom (2,3 %) und eine schmerzlose skrotale Schwellung (2,3 %). Auch im
Kindes- und Jugendalter ist die antegrade Sklerotherapie eine erfolgreiche minimal-invasive
Methode zur ambulanten Behandlung der Varikozele. Der schonende, nebenwirkungsarme
Eingriff kann bereits in der frühen Pubertät prophylaktisch vorgenommen werden, um
testikulären Schäden vorzubeugen.
Abstract
Antegrade scrotal sclerotherapy was performed as an out-patient treatment in 40 boys,
10 to 15 years old, with varicocele testis. 36 boys had left-sided, 4 boys had bilateral
varicoceles. 18 varicoceles were grade III (40.9 %), 22 grade II (50 %) and 4 grade
I (9.1 %). Of 44 treatments, 29 were carried out under local anaesthesia, 15 were
done under lumbar anaesthesia. At follow up (range 1-27 months, mean 9.1 months),
1 persisting varicocele (III°) was detected and successfully treated by repeated sclerosation
(persistence rate 2.3 %). Complications included one boy with a local hematoma (2.3
%) and one boy with a painless scrotal swelling (2.3 %). In children and adolescents,
varicocele can successfully be treated by the minimally invasive method of antegrade
sclerotherapy on an out-patient basis. Since treatment is easy on the patient and
has a low complication rate, correction of varicocele might be carried out prophylactically
in early puberty to prevent testicular lesions.
