Zusammenfassung
Bei einem 41jährigen Patienten im unklaren Kreislaufschock (systolischer Blutdruck
50 mm Hg) fielen eine ausgeprägte Rötung des Oberkörpers sowie makulopapulöse Effloreszenzen
am ganzen Körper auf. Nach Infusion von 1500 ml einer kolloidalen Lösung stieg der
Blutdruck so weit an, daß das Befinden nicht mehr beeinträchtigt war. Der Patient
gab an, in den letzten Jahren bereits ähnliche Anfälle von Flush und Kreislaufkollaps,
jeweils einen Tag nach Konsum größerer Alkoholmengen (3-41 Bier), erlitten zu haben.
Die klinisch-chemische Untersuchung ergab einen Quick-Wert von 56 %, eine partielle
Thromboplastinzeit von 130 s und eine Thrombinzeit > 180 s. 3 Stunden später hatten
sich die Gerinnungszeiten weiter verschlechtert, innerhalb von 20 Stunden wurden jedoch
wieder Normwerte erreicht. Unter Ruhebedingungen und nach Provokation (heißes Bad)
ergaben sich folgende Serumkonzentrationen: Heparin 0,21 E/ml und 0,85 E/ml; Histamin
1,9 µg/ml und 2,3 µg/ml; Serotonin 23 µg/ml und 38 µg/ml. Die histologische Untersuchung
eines Beckenkammtrepanates ließ dichte Ansammlungen von Mastzellen erkennen, entsprechend
einer systemischen Mastozytose. Das Hautbiopsat ergab die Diagnose einer Urticaria
pigmentosa.
Abstract
A 41-year-old man was admitted in circulatory shock of unknown aetiology (systolic
pressure 50 mm Hg) and marked reddening of the upper part of the body as well as maculopapular
rash over the whole body. After 1500 ml of colloidal solution had been infused the
blood pressure rose to a level at which the patient's condition was no longer at risk.
He reported having had a similar attack of flushing and circulatory collapse during
the last few years, each time after drinking 3-41 of beer. Laboratory tests showed
thromboplastin time 56 %, partial thromboplastin time 130 s and thrombin time > 180 s.
Three hours after admission the coagulation times had further deteriorated, but had
become normal within 20 hours. After rest and after a provocation (hot bath) the serum
concentrations were: heparin 0.21 U/ml and 0.85 U/ml; histamine 1.9 µg/ml and 2.3 µg/ml;
serotonin 23 µg/ml and 38 µg/ml. Histological examination of an iliac crest bone marrow
biopsy revealed dense collections of mast cells, as seen in systemic mastocytosis.
A skin biopsy was diagnostic of urticaria pigmentosa.
