Urethraplastik zum Ersatz der hinteren weiblichen Harnröhre

Zusammenfassung

Der vollständige Verlust von Blasenhals und proximaler Urethra bei einem Mädchen infolge einer Beckenfraktur durch Kriegsverletzung wurde durch Bildung eines Blasenrezessus aus dem Blasenboden überbrückt (“umgekehrter Psoas-Hitch”). Trotz fehlender sphinktärer Verschlußmechanismen konnte volle Harnkontinenz allein durch eine korrekte anatomische Lage der Neourethra in einem durch Lyo-Dura-Plastik und vorderer Scheidenplastik gefestigtem Beckenboden erzielt werden. Das gute postoperative Ergebnis bestätigte sich sowohl hinsichtlich einer ungestörten Miktion wie auch einer weiterhin vollständigen Kontinenz bei einer Nachuntersuchung nach 12 Monaten.

Abstract

War injury in a 15-year-old girl caused a fracture of the anterior pelvis followed by the complete loss of the bladder neck and the proximal urethra. The missing link was built by a recessus from the bladder bottom. In spite of the missing intrinsic sphincteric mechanisms full urinary continence could be achieved by correct anatomic positioning of the neourethra and by tightening the pelvic floor with a sling procedure using lypohilisized dura and the kolporrhaphia anterior. The one year follow-up examination confirmed the excellent results concerning micturition without any complaints and full urinary continence.
