Zusammenfassung
Das detubularisierte rechte Hemikolon wird sowohl zum Reservoir bei Kontinenter Harnableitung
als auch zum Blasenersatz verwendet. Bei 21 Patienten wurde eine kontinente Harnableitung,
bei 22 Männern ein Blasenersatz vorgenommen. Bei 4 Patienten mit kontinenter Ableitung
waren weitere Eingriffe notwendig (perkutane Nephrostomie, Wundrevision, Nippelkorrektur).
4 der 22 Patienten mit Blasenersatz hatten Komplikationen wie Ileus, Wunddehiszenz,
Candidasepsis. Von 2 Patienten mit Blasenersatz abgesehen, waren alle kontinent. Die
Funktion des oberen Harntraktes wird durch Verwendung des rechten Hemikolons nicht
stärker beeinflußt als bei Harnableitung über ein Conduit. Die beschriebene Technik
eignet sich sowohl zur kontinenten Harnableitung als auch zum Blasenersatz.
Abstract
The detubularized right colonic segment is used as urinary reservoir in continent
diversion and as bladder substitute. 21 patients underwent continent urinary diversion
and 22 male patients got a bladder substitute. In 4 of the patients with continent
urinary diversion reoperation was necessary (percutaneous nephrostomy, wound revision,
revision of the nippel). 4 of the 22 patients with bladder substitute had complications,
that is small-bowel obstruction, wound dehiscence and Candida septicemia. Except for
2 men with bladder substitute all patients achieved continence. The use of the right
colon as urinary reservoir does not endanger the function of the upper urinary tract
more than urinary diversion by a conduit. The clinical results of these methods have
been excellent.
