Zusammenfassung
Ein 66jähriger Patient wies ein Plattenepithelkarzinom der Harnblase des prätherapeutischen Stadiums T4 C2 N3 M0 auf. Nach Downstaging mit Rückbildung der computertomographisch vergrößerten pelvinen Lymphknoten durch 2 Zyklen einer Polychemotherapie (M-VAC) erfolgte die radikale Zystektomie sowie pelvine und retroperitoneale Lymphadenektomie mit Entfernung eines 8 cm großen retroperitonealen Tumors, der sonographisch als Lymphknotenmetastase mit zentralen Nekrosen, histologisch aber als Angioleiomyom bestimmt wurde. Dies und das postchirurgische Tumorstadium pT2 C4 N0 M0 gibt Anlaß, die Wirksamkeit des M-VAC Schemas beim Plattenepithelkarzinom, die Treffsicherheit bildgebender Verfahren und das Zusammentreffen eines Angioleiomyoms mit einem Harnblasenkarzinom zu diskutieren.
Abstract
A 66 year old male patient presented himself with a squamous cell carcinoma of the bladder. Based on IVP, abdominal sonography and computertomography the tumour was staged T4 C2 N3 M0 with a large left sided retroperitoneal mass and pelvic lymph nodes. Locoregional downstaging was successfully achieved with preoperative M-VAC chemotherapy which was followed by radical cystectomy, pelvic and abdominoretroperitoneal lymph node dissection and ileum conduit. The retroperitoneal mass which - based on sonography - was believed to be a metastasis, was in fact an angioleiomyoma. The pelvic nodes were free of tumour and local tumour mass showed a significant reduction: The postsurgical tumour stage was pT2 C4 N0 M0. The efficacy of chemotherapy in squamous cell carcinoma, the accuracy of modern imaging techniques and the frequency of angioleiomyoma in combination with bladder cancer are discussed.
