Aktinomykose der Harnblase kompliziert durch eine Antibiotika-Allergie

Zusammenfassung

Es wird über den seltenen Fall einer Aktinomykose der Harnblase berichtet, der zunächst als Blasentumor mißgedeutet wurde. Nach TUR und damit histologischer Diagnose wurde die Patientin mit 2 × 10 Mega Penicillin G behandelt. Nach 2 Wochen entwickelte sich jedoch eine Allergie auf Penicillin und das Alternativpräparat Minocyclin, so daß die Behandlung schließlich mit Cefoxitin als Antibiotikum der dritten Wahl erfolgen mußte. Der ätiologische Zusammenhang zwischen einem Intrauterinpessar und der Entwicklung der Aktinomykose wird diskutiert.

Abstract

We report on a rare case of actinomycosis of the bladder being primarily suspected as a bladder tumour. After TUR and microscopic diagnosis the patient was treated antibiotically with 2 × 10 Mega Penicillin G. Following two weeks allergical reaction to Penicillin as well as to the alternative drug Minocyclin occurred. Thus the treatment had to be continued using Cefoxitin as antibiotic of 3rd choice. The etiological correlation between intrauterine device (IUD) and actinomycosis is discussed.
