Zusammenfassung
Ein Karzinom der rechten Niere mit Tumorthrombus in der Vena cava bis über die Lebervenenmündung wurde unter Einsatz des extrakorporalen Kreislaufes (EKK) erfolgreich operiert. Vorhoftumorthromben sind nur mit Anwendung des EKK entfernbar; bei großen Tumorthromben, die in die Pars hepatica der Hohlvene reichen, wird die Radikalität und Sicherheit des Eingriffes durch den EKK erhöht. Bei Totalverschluß der Hohlvene durch Tumorthrombus ist die Abklemmung der Arteria hepatica und der Vena portae vor der temporären intraperikardialen Okklusion der Hohlvene zu empfehlen, um venöse Stase der Leber während des EKK und retrograde Blutung nach Tumorausräumung zu verringern.
Abstract
A carcinoma of the right kidney with massive tumour thrombus extending above the hepatic vein entry was successfully operated by the use of extracorporeal circulation (ECC). Tumour thrombus extending to the right atrium may requires the application of ECC for removal; with tumour thrombus of the hepatic portion of the cava, ECC increases the safety and radicality of the procedure. When the inferior vena cava is totally occluded by tumour thrombus, clamping of the hepatic artery and portal vein prior to temporary occlusion of the intrapericardial vena cava is recommended to decrease venous stasis of the liver and to diminish retrograde bleeding following removal of the intracaval tumour.
