Urologische Spätkomplikationen nach kurativer gynäkologisch-radiologischer Karzinomtherapie

Zusammenfassung
Von 214 Patientinnen mit gynäkologischem Karzinom, vorangegangener Strahlenmonotherapie bzw. adjuvanter Irradiatio, wurden 37 rezidivfreie Patienten mit Strahlenspätkomplikationen nachuntersucht.
Die einzelnen Komplikationen wie Strahlenblase, Ureterstenose und Blasenfistel werden mit ihren therapeutischen Möglichkeiten angegeben und besprochen. Besondere Beachtung verdient die hohe Mortalitätsrate 5/37 (14%) von kurativ radiologisch behandelten Patientinnen, die nach den Richtlinien der UICC mit einer 5-Jahres-Überlebenszeit als geheilt in die Statistik eingingen, jedoch an den Folgen der Therapie nach mehr als 5 Jahren verstarben.
Abstract
37 patients without recurrency, out of a total of 214 who were treated with radiation only or adjuvant irradiation after gynecologic cancer, were followed up because of late complication from irradiation.
Complications like actinic cystitis, stenosis of the ureter and vesical fistula were listed up and discussed including therapeutic possibilities. Special notice deserves the high rate of mortality (14%) in patients with curative actinic treatment, who, following the standards of UICC, are statistically claimed to be cured with a survival time of 5 years, but died from late complications of therapy after more than 5 years.
