Zusammenfassung

Ein 4jähriges Mädchen mit rechtsseitigem Wilms-Tumor und Tumorzapfen bis in den rechten Vorhof wurde erfolgreich radikal operiert und lebt heute 22 Monate nach dem Eingriff tumorfrei. 13 gleichartige Fälle wurden bis 1981 aus der Literatur bekannt. Überlebenszeiten, diagnostische Besonderheiten und ihre therapeutischen Konsequenzen werden verglichen. Entscheidend ist die präoperative Kenntnis der proximalen Ausdehnung des Tumorzapfens. Sie erst ermöglicht eine gezielte Operationsstrategie und damit einhergehend ein gut kontrollierbares Operationsrisiko.

Abstract

This is a report of a 4-year old girl with a Wilms tumor of the right kidney extending through the inferior vena cava to the right atrium. The tumor and its thrombus were removed completely and the patient is alive and well 22 months postoperatively without recurrence. Surgical problems and results are compared with the 13 similar cases published until 1981. The knowledge of the exact tumor extension into the vena cava and the atrium is essential for planning and performing a successful operation.
