Zusammenfassung
Vor elektiven urologischen Operationen mit Eröffnung des Dickdarmes muß das Kolon vorbereitet werden mit dem Ziel, die Stuhl- und Keimmenge im Darm zu reduzieren. Von entscheidender Bedeutung ist die mechanische Reinigung, was durch Gabe von Astronautenkost allein, Verabreichung von Laxantien und Einläufen oder durch orthograde Darmspülung erreicht werden kann. Die orthograde Darmspülung bietet sich als Methode der Wahl an, da bei den urologischen Patienten in über 90% der gesamte Intestinaltrakt in wenigen Stunden leergespült werden kann. Für das Gelingen der Spülung müssen einige Punkte beachtet werden, die in der Arbeit aufgrund eigener Erfahrungen abgehandelt werden. Die Bedeutung einer darmkeimspezifischen Antibiotika-Prophylaxe kann beim urologischen Krankengut wegen fehlender klinischer Studien nicht gewertet werden.
Abstract
Before elective urological operations involving opening of the colon, preparation of the intestines must first be accomplished with the aim of reducing the amount of feces and bacteria. Of decisive importance is mechanical purgation, which can be achieved by means of a diet composed solely of astronaut food, laxatives and enemas, or by whole-gut irrigation. Whole-gut irrigation is the method of choice, as, in over 90% of urological patients, the entire intestinal tract can be rinsed clean in only a few hours. Several points which have proven important for the success of irrigation are discussed. The significance of intestinal bacteria-specific antibiotic prophylaxis cannot be evaluated in this urological patient material due to a lack of clinical studies.
