Knochenbruch und Gefäßverletzung beim polytraumatisierten Kind

Abstract

The combination of fractures and simultaneous vascular lesions is not so rare as is generally thought.
A direct, sharp vascular injury can easily be recognised by the symptoms, whereas blunt injuries to vessels, or indirect lesions, are often disguised.
The vessels at the back of the knee have a special significance for anatomical reasons. - The complete and incomplete peripheral ischaemia syndrome is described with regard to the important therapeutic consequences to the skeletal muscles.
Our diagnostic and therapeutic procedure is described with numerous illustrations, on the basis of an open fracture of the lower leg (second degree), which was associated with a third-degree injury to the back of the knee and blockage of the vessels.

Zusammenfassung

Die Kombination von Knochenbrüchen mit gleichzeitiger Gefäßläsion ist nicht so selten wie allgemein angenommen wird.
Ein direktes, scharfes Gefäßtrauma kann durch seine Symptomatik leicht erkannt werden, während stumpfe Gefäßverletzungen bzw. indirekte Gefäßläsionen häufig kaschiert sind.
Die Gefäße der Kniekehlenregion nehmen aus anatomisch-topographischen Gründen eine Sonderstellung ein; das vollständige und unvollständige periphere Ischämie-Syndrom wird im Hinblick auf die wichtige therapeutische Konsequenz für die quergestreifte Skelettmuskulatur dargestellt.
Unser diagnostisches und therapeutisches Vorgehen wird bei einer zweitgradig offenen Unterschenkelfraktur, die mit einer drittgradig offenen Kniekehlenverletzung und gleichzeitigem Gefäßverschluß einherging, anhand zahlreicher Abbildungen aufgezeigt.
