Abstract
The aim of this study was to determine the role of surgery in the treatment of abdominal B-cell non-Hodgkins lymphomas (B-NHL) in children. We analyzed the effect of surgical variables of initial laparotomy and second-look surgery on event-free survival (EFS) of 177 patients with abdominal B-NHL enrolled into the three consecutive multicenter trials NHL-BFM 81, NHL-BFM 83, and NHL-BFM 86. The therapy regimen was comparable in all 3 trials as well as the overall outcome of the patients. Patients with stage II and complete resection received 3 courses of therapy (4 in trial NHL-BFM 81), patients with stage II not resected, stage III, and stage IV received 6 courses of therapy (8 in trial NHL-BFM 81). An initial laparotomy was performed in 161 patients, in 59 of them as an urgent procedure. Complete resection of the abdominal primary was performed in 43 patients, 40 of them had a localized bowel tumor. The probability of EFS (pEFS) at 5 years is 95%, 69%, 62%, and 67% for patients with complete resection, subtotal resection (n = 36), partial resection (n = 21), or biopsy only (n = 61), respectively. Complete resection was achieved in 30 out of 40 patients with stage II, but only in 12 of 113 and 1 of 24 patients with stage III and IV, respectively. pEFS at 5 years according to stage and completeness of resection is as follows: stage II complete resected 97%; stage II not complete resected 100%; stage III/IV complete resected 92%; stage III/IV not complete resected 63%. The rate of postoperative complications (n = 20) was 19% after incomplete resections and 7% in patients with a biopsy only. Two patients suffered from relapse along a former drainage tube channel. Second-look surgery was performed in 60 patients. The intraoperative findings corresponded to the preoperative findings of ultrasonography and/or computed tomography in 88% of the cases. The rate of failures after the second-look operation was 9 of 27 patients without residual tumor, 4 of 25 patients with residual tumors containing only necrotic tissue, and 5 of 8 patients with active residual lymphoma. Our conclusions are: For patients with localized bowel tumors complete removal is of advantage allowing a reduced chemotherapy. However, no major functional impairment is justified since an equally good prognosis is achieved with intensified chemotherapy. In patients with extended disease, a biopsy only is recommended; debulking procedures are to be avoided. Drainage tubes are not to be used. If the diagnosis can be established from examination of effusions or the bone marrow, a laparotomy should be avoided so that chemotherapy can be started without delay. Even in patients presenting as a surgical emergency a preoperative ultrasonography should be performed in order to give the surgeon valuable information for planning the appropriate procedure. Second-look surgery is not needed in addition to ultrasonography or CT for detection of residual tumors after initial chemotherapy. However, a second-look operation is still of value in order to identify those patients with vital residual tumors who may benefit from an intensified treatment.
Zusammenfassung
Die Zielsetzung dieser Arbeit ist die Definition der Rolle chirurgischer Interventionen in der Behandlung abdominaler B-Zell-Non-Hodgkin-Lymphome (B-NHL) im Kindesalter. Wir analysierten den Effekt chirurgischer Variablen der initialen Laparotomie und der Second-Look-Operation auf das ereignisfreie Überleben (EFS) von 177 Patienten, die in die 3 aufeinanderfolgenden multizentrischen Studien NHL-BFM 81, NHL-BFM 83 und NHL-BFM 86 aufgenommen wurden. Das Chemotherapieprotokoll war in allen 3 Studien vergleichbar. Patienten mit Stadium II und vollständiger Resektion des Lymphoms erhielten 3 Therapiekurse, alle anderen erhielten 6 Kurse (4 bzw. 8 Kurse in Studie 81). Das Gesamtergebnis aller 3 Studien ist vergleichbar. Bei 161 Patienten wurde eine primäre Laparotomie durchgeführt, bei 59 Patienten als Notfalloperation. Bei 43 Patienten wurde eine vollständige Resektion des abdominalen Tumors erreicht, 40 von ihnen hatten einen lokalisierten Darmtumor. Die Wahrscheinlichkeit des EFS (pEFS) nach 5 Jahren ist 95%, 69%, 62% und 67% für Patienten mit initial vollständiger Resektion, subtotaler Resektion (n = 36), partieller Resektion (n = 21) oder Biopsie allein (n = 61). Bei 30 von 40 Patienten mit Stadium II wurde eine vollständige Resektion erreicht, aber nur bei 12 von 113 und 1 von 24 Patienten mit Stadium III oder IV respektive. Nach Stadium und Vollständigkeit der Resektion ist pEFS nach 5 Jahren wie folgt: Stadium II komplett reseziert 97%; Stadium II nicht komplett reseziert 100%; Stadium III/IV komplett reseziert 92%; Stadium Ill/IV nicht komplett reseziert 63%. Die Rate postoperativer Komplikationen (n = 20) war am höchsten nach unvollständiger Resektion (19%) und am geringsten bei Patienten mit Biopsie allein (7%). Zwei Patienten erlitten ein Rezidiv im Stichkanal einer Ablaufdrainage. Bei 60 Patienten wurde eine Second-Look-Operation durchgeführt. Bei 88% dieser Patienten entsprach der intraoperative Befund dem präoperativen Befund der Sonographie und/oder Computertomographie. Neun von 27 Patienten ohne intraoperativ erkennbaren Resttumor erlitten nachfolgend ein Rezidiv, 4 von 25 Patienten mit nekrotischem Resttumor und 5 von 8 Patienten mit vitalem Resttumor. Unsere Schlußfolgerungen sind: Bei Patienten mit lokalisiertem Darmbefall ist eine vollständige Resektion unter Mitnahme verdächtiger Mesenteriallymphknoten sinnvoll. Resektionen mit funktionellem Verlust sind aber nicht gerechtfertigt, da die Patienten auch bei unvollständiger Resektion mit einer intensivierten Chemotherapie gleich gute Überlebenschancen haben. Bei Patienten mit retroperitonealem oder disseminiertem abdominalem Befall sollte sich der Eingriff auf eine ausreichende Biopsie beschränken. Erweiterte Resektionen sind ohne Wert und mit einer höheren Komplikationsrate belastet. Wenn bei diesen Patienten die Diagnose aus der Untersuchung von Knochenmark, Aszites oder Pleuraerguß gestellt werden kann, sollte auf die Laparotomie ganz verzichtet und die Chemotherapie rasch eingeleitet werden. Drainagen sollten vermieden werden. Auch bei Patienten, die als chirurgischer Notfall imponieren, sollte eine präoperative Sonographie erfolgen, um das geeignete Vorgehen zu planen. Eine Second-Look-Operation ist nur dann indiziert, wenn nach ausreichender Chemotherapie die bildgebende Diagnostik einen Resttumor vermuten läßt, um zwischen einem nekrotischen Restbefund und vitalem Restlymphom zu unterscheiden. Patienten mit vitalem Restlymphom hatten eine schlechte Prognose und benötigen einen effektiveren Therapieansatz.
