The Diagnostic and Therapeutic Use of Colonoscopy in Infancy and Childhood

Abstract
During the period October 1972 to May 1979, 76 colonoscopies were carried out on children between the ages of 5 months and 15 years.
Ulcerative colitis/Mb. Crohn has been the indication for colonoscopy in 33 children. More information in order to confirm or repudiate the diagnosis was required in 17 children, severity and extent of changes determined or local effect of drug therapy evaluated in 16 cases.
Colonoscopy was able to explain the bleeding in 10 of 12 children with unexplained rectal haemorrhage and to stop bleeding in 5 cases.
Among the remaining 15 children with suspected or by X-ray demonstrated colonic polyp(s) 10 had the diagnosis confirmed and the polyp(s) removed by colonoscopic polypectomy in the same session. In the remaining 5 cases the suspicion was not confirmed
Zusammenfassung
In der Zeit von Oktober 1972 bis Mai 1979 wurden 76 Koloskopien bei Kindern im Alter von 5 Monaten bis 15 Jahren vorgenommen.
Bei 33 Kindern war eine Colitis ulcerosa/Morbus Crohn die Indikation für die Koloskopie. Bei 17 Kindern war mehr Information notwendig, um die Diagnose zu bestätigen oder auszuschließen. Bei 16 Fällen wurden der Schweregrad und die Ausdehnung der Veränderungen oder der lokale Effekt der medikamentösen Behandlung bestimmt.
Durch die Koloskopie konnte bei 10 von 12 Kindern eine rektale Blutungsquelle gefunden und in 5 Fällen die Blutung zum Stehen gebracht werden.
Unter den verbleibenden 15 Kindern mit Verdacht auf oder röntgenologisch nachgewiesene Polypen wurde bei 10 die Diagnose bestätigt und der bzw. die Polypen in derselben Sitzung koloskopisch entfernt. Bei den verbleibenden 5 Fällen wurde der Verdacht nicht bestätigt.
