Alkoholabusus als mögliche Ursache eines Holiday-Heart-Syndroms und tiefer Bewusstlosigkeit

Zusammenfassung

Ein 29-jähriger Mann wird in seiner Wohnung tief bewusstlos (Glasgow-Coma-Scale von 3) aufgefunden. Neben einer Hypoxie imponierte eine Tachyarrhythmia absoluta. Nach Intubation wurde der Patient kreislaufinstabil und musste einmalig elektrisch kardiovertiert werden. Als mögliche Ursachen der Bewusstlosigkeit wird differenzialdiagnostisch ein äthyltoxisch induzierter zerebraler Krampfanfall sowie eine hämodynamische Instabilität im Rahmen eines „Holiday-Heart-Syndroms” diskutiert. Der Patient konnte noch am Aufnahmetag ohne neurologisches Defizit extubiert werden.

Abstract

A 29 year old man was found comatose (Glasgow coma scale 3), hypoxic and with a tachyarrhythmia in his room. After intubation the patient developed circulatory disturbance and a cardioversion was performed. As possible reasons of the coma an alcohol induced seizure as well as a hemodynamic disturbance due to a holiday heart syndrome were discussed. The patient was extubated at the same day without a neurological deficit.
