Zusammenfassung
In einer prospektiven Therapiestudie wurde die perinatale Morbidität von 141 normalen Schwangerschaften (Gruppe I), von 108 Schwangerschaften mit frühzeitig behandeltem Gestationsdiabetes (Gruppe II) und von 35 Patientinnen mit ungenügend behandeltem Gestationsdiabetes (Gruppe III) untersucht. Das Therapieziel bei Gestationsdiabetes war eine postprandiale Blutglucosekonzentration unter 130 mg/dl; war dieses Ziel durch Diabetes-Diät allein nicht erreichbar, wurde einmal täglich Insulin injiziert. Gruppe II zeigte eine normale Häufigkeit von neonataler Makrosomie Dystrophie, Acidose, Hyperbilirubinämie, Hypoglykämie und Hypocalcämie. Hingegen war in der ungenügend behandelten Gruppe III die Makrosomie mehr als dreifach häufiger als in Gruppe II (P < 0,02) und eine Acidose (pH < 7,20) doppelt so häufig (P < 0,05) wie in Gruppe II. Eine strikte Stoffwechseleinstellung führt also bei Gestationsdiabetes zu einer Normalisierung der perinatalen Morbidität.
Abstract
A prospective study compared the perinatal morbidity of 141 normal pregnancies (group I) with that of 108 pregnancies in whom gestational diabetes had been treated early (group II) and 35 with unsatisfactorily treated gestational diabetes (group III). The therapeutic goal in gestational diabetes was to have a postprandial blood-glucose level of less than 130 mg/100 ml. If this was not achievable through diet alone, insulin was injected once daily. Neonatal macrosomia, dystrophy, acidosis, hyperbilirubinaemia, hypoglycaemia and hypocalcaemia had a normal incidence in group II, but in group III macrosomia was twice as frequent as in group II (P < 0.02), and acidosis (pH < 7.20) twice as frequent (P < 0.05). The results indicate that strict metabolic control in gestational pregnancy will achieve a normal rate of perinatal morbidity.
