Zur Problematik des idiopathischen Chylothorax^*

Zusammenfassung

Bei einem 57jährigen Patienten fand sich als Ursache rechtsseitiger atemabhängiger Thoraxschmerzen ein Chylothorax. Eine dreiwöchige konservative Behandlung, bei der insgesamt 25 Liter Chylus entleert wurden, führte nicht zur Besserung. Nach Thorakotomie mit Ligatur und Resektion des Ductus thoracicus kam es zu sofortigem Sistieren des Lymphflusses. Weder eine umfangreiche präoperative Diagnostik noch die histologische Untersuchung des Lymphganges konnten die Krankheitsurache klären.

Abstract

Conservative management of a chylothorax in a 57-year-old man was unsuccessful (25 l chylous fluid removed over three weeks). But chylous flow ceased immediately after thoracotomy with ligation and resection of the thoracic duct. The aetiology of the chylothorax remained obscure despite extensive preoperative diagnostic measures and histological examination of the thoracic duct.
