Plasmozytome ohne Paraprotein

Zusammenfassung

Die seltenen Plasmozytome ohne Paraprotein (Häufigkeit weniger als 1 %) können eine diagnostische Fallgrube darstellen. Drei eigene Beobachtungen geben Anlaß, auf Fehlinterpretationen der Symptome und die Problematik bei der Diagnosestellung hinzuweisen. Bei einem Patienten war ein monoklonales Immunglobulin (kappa-Leichtketten) in den Myelomzellen des Knochenmarks nachweisbar (nicht-exkretorisches Plasmozytom). Bei einem weiteren Patienten konnte trotz mehrfacher Untersuchung kein Paraprotein in den Myelomzellen gefunden werden (nicht-sekretorisches Plasmozytom). Beim dritten Patienten lag als weitere Besonderheit gleichzeitig eine chronische lymphatische Leukämie vor; sein Plasmozytom wurde erst autoptisch nachgewiesen.

Abstract

Rare plasmocytomas without paraprotein (frequency less than 1 %) can present a diagnostic trap. Three observations give grounds for drawing attention to misinterpretation of symptoms and the problematic nature of the diagnosis. In one patient, a monoclonal immunoglobulin (kappa light-chains) was detectable in the bone-marrow myeloma cells (non-excretory plasmacytoma). In a further patient no paraprotein could be found in the myeloma cells (non-secretory plasmacytoma) despite repeated investigations. An additional unusual feature in a third patient was the simultaneous presence of chronic lymphocytic leukaemia; his plasmocytoma was first detected at autopsy.
