Therapiemöglichkeiten beim Hypereosinophilie-Syndrom mit Endomyocarditis fibroplastica Löffler: Wirksamkeit von Cytarabin und 6-Thioguanin

Zusammenfassung

Bei zwei von vier Patienten mit Hypereosinophilie-Syndrom und bioptisch gesicherter Endomyocarditis fibroplastica Löffler ließ sich die Krankheitsaktivität mit Prednison, in einem Fall kombiniert mit Hydroxycarbamid, beherrschen. Bei zwei weiteren Patienten mit aggressivem Verlauf waren nach Versagen der Vorbehandlung Cytarabin und 6-Thioguanin wirksam. Damit ist eine Patientin seit 16 Monaten beschwerdefrei. Der zweite Patient starb infolge Sepsis bei therapie-induzierter Knochenmarkhypoplasie. Prednison (1 mg/kg täglich) und Hydroxycarbamid (0,5-1,5 g täglich) sind beim Hypereosinophilie-Syndrom die Mittel der Wahl. Bei Versagen dieser Substanzen sollten Cytarabin (100 mg/m² Tag 1-5, Wiederholung Tag 28) und 6-Thioguanin (100 mg/m² täglich) eingesetzt werden.

Abstract

In two of four patients with hypereosinophilia and Löffler's myocarditis (confirmed by biopsy) the activity of the disease was contained with prednisone, in one instance combined with hydroxycarbamide. In two patients, in whom the disease was taking a fulminant course and other treatment had failed, cytarabine and 6-thioguanine proved effective, providing an 16-month symptom-free period in one of them. But the second patient died from septicaemia associated with treatment-induced bone-marrow hypoplasia. Prednisone (1 mg/kg daily) and hydroxycarbamide (0.5-1.5 g daily) are the drugs of choice in the hypereosinophilia syndrome. If they fail, cytarabine (100 mg/m² on days 1-5, repeated on day 28) and 6-thioguanine (100 mg/m² daily) should be given.
