Atrioventrikulärer Block nach Carotisdruck bei Hypothyreose: Normalisierung nach Substitution

Zusammenfassung

Bei einer 79jährigen Patientin fand sich eine schwere Autoimmunhypothyreose, wahrscheinlich nach Hyperthyreose bei M. Basedow (T₃ und T₄ unmeßbar tief), ohne die klassischen Zeichen des Myxödemherzens. Als Besonderheiten konnten beim Carotissinus-Druckversuch reproduzierbar das passagere Auftreten eines AV-Blockes ersten Grades sowie eine einmalige vollständige Blockierung nachgewiesen werden, was bei einer Kontrolluntersuchung nach vierwöchiger Substitutionsbehandlung unter 50 µg L-Thyroxin nicht mehr der Fall war. Eine weitere Kontrolle nach einem halben Jahr unter 75 µg Thyroxin ergab wiederum einen negativen Carotissinus-Druckversuch.

Abstract

Severe auto-immune hypothyroidism, probably after hyperthyroidism, was diagnosed in a 79-year-old woman with hypothyroidism (T₃ and T₄ immeasurably low) without the classical signs of myxoedema heart. On carotid sinus pressure transitory first degree AV block occurred and, on one occasion, complete AV block. This response disappeared after substitution treatment for four weeks with 50 µg L-thyroxine. Further examination half a year later, on a maintenance of 75 µg thyroxine, also gave a negative response to carotid sinus pressure.
