Zusammenfassung

Postoperative pulmonale Komplikationen gehören zu den Hauptfaktoren postoperativer Morbidität und Mortalität. Raucheranamnese, COPD, offene Oberbauchchirurgie und Thorakotomie gehören zu den Hauptrisikofaktoren. Die Prädiktion des Risikos für PPC kann im Rahmen der Anamneseerhebung und klinischen Untersuchung valide erfolgen. Erweiterte, organspezifische Funktionsprüfungen sind beim Vorliegen pulmonaler Symptome indiziert.

Die Prophylaxe der PPC muss präoperativ Raucherentwöhnung wenn möglich, ein gutes Infektionsmanagement, Optimierung einer antiobstruktiven und kardioprotektiven Therapie, Ausgleich von Mangelzuständen einschließlich bedarfsgerechter Transfusion beinhalten.

Intraoperativ sind vor allem bei bronchialer Hyperreagibilität Regionalanästhesieverfahren, insbesondere auch zur postoperativen Akutschmerztherapie von Vorteil. Postoperativ kommt es besonders bei COPD auf eine frühe Extubation an, gegebenenfalls gefolgt von NIV, suffiziente Lagerung, Entlastung des Abdomen durch Abführmaßnahmen und Magensonde, eine frühe enterale Ernährung und Frühmobilisierung an.

Abstract

Postoperative pulmonary complications (PPC) are considered a main factor increasing postoperative morbidity and mortality. Medical history and clinical examination are highly predictive for the development of PPC. Extended evaluation is indicated in the presence of pulmonary symptoms. Prophylaxis of PPC includes cessation of smoking, antiinfectious measures, optimization of bronchodilator therapy and improvement of cardiac performance. Regional anesthesia is favorable in the presence of bronchial hyperreactivity and for postoperative pain therapy. Postoperative course includes early extubation besides positioning, relief of intrabdominal pressure and physiotherapy.

Postoperative pulmonale Komplikationen gehören zu den Hauptfaktoren postoperativer Morbidität und Mortalität.
Zu den Hauptrisikofaktoren zählen Raucheranamnese, COPD, offene Oberbauchchirurgie und Thorakotomie.
Das Risiko für PPC kann im Rahmen der Anamneseerhebung und klinischen Untersuchung valide abgeschätzt werden.
Bei Vorliegen pulmonaler Symptome sind Lungenfunktionsuntersuchungen, Blutgasanalyse und Belastungstests, immer aber auch eine suffiziente kardiale Evaluation indiziert.
Die Prophylaxe der PPC muss präoperativ möglichst Raucherentwöhnung, ein gutes Infektionsmanagement, Optimierung der antiobstruktiven und kardioprotektiven Therapie, Ausgleich von Mangelzuständen einschließlich bedarfsgerechter Transfusion beinhalten.
Intraoperativ sind vor allem bei bronchialer Hyperreagibilität Regionalanästhesieverfahren – insbesondere auch zur postoperativen Akutschmerztherapie – anzuwenden.
Die suffiziente Akutschmerztherapie fördert die frühzeitige Mobilisierung post OP und ist somit wichtig für die Prophylaxe der PPC.
Frühe Mobilisierung beugt hypostatischen Atelektasen, Thrombosen, Ödemen, Muskelatrophie und Darmatonien am besten vor und ermöglicht eine effektive Mukolyse.
Grundsätzlich ist nach der Operation eine frühzeitige Extubation anzustreben. Gerade COPD–Patienten sollten extubiert werden, sobald Schutzreflexe, Vigilanz und eine geringe Aspirationsgefahr dies zulassen.
