Abstract
Pediatric intussusception is usually idiopathic in origin, and a pathological lesion at the leadpoint is identified in only a small proportion of cases. Meckel's diverticulum is the most common location when such a lesion is found. Heterotopic pancreas has been seen most frequently in the stomach or duodenum; however, ileal lesions are rare. In the ileum, heterotopic pancreas is usually associated with Meckel's diverticulum and may cause intestinal obstruction by intussusception. However, isolated heterotopic pancreas in the ileum is distinctly rare and usually discovered incidentally during surgery for other conditions. In a review of English language literature, there were only a few pediatric cases of isolated ileal heterotopic pancreas causing intussusception. We report three additional pediatric cases and discuss the clinical significance of this unexpected rare entity.
Zusammenfassung
Die Invagination ist im Kindesalter keine seltene Erkrankung. Häufig findet man jedoch keine organische Ursache der Einstülpung. Am häufigsten werden Meckelsche Divertikel beobachtet, jedoch auch diese nur in ca. 10% der Fälle. Sehr ungewöhnlich, wenn auch bekannt, ist das Vorkommen eines heterotopen Pankreas. Man findet heterotopes Pankreasgewebe häufig im Bereich des Magens und Duodenums, jedoch selten im Bereich des lleums. Hier ist es oft mit einem Meckelschen Divertikel kombiniert. Noch seltener ist das Vorkommen von heterotopem Pankreasgewebe ohne Meckelsches Divertikel. Da isoliertes, heterotopes Pankreasgewebe zwar in Lehrbüchern immer wieder als Ursache einer Invagination beschrieben wird, jedoch kasuistische Beschreibungen in der Literatur praktisch nicht vorkommen, werden in der vorliegenden Arbeit 3 Fälle einer Invagination auf Grund heterotopem Pankreasgewebes vorgestellt und die Literatur diskutiert.
