Diagnostische Kriterien und Klassifikation
Zentrale Symptome der AN und der BN sind eine ausgeprägte Angst vor Gewichtszunahme („Gewichtsphobie”), restriktives Essverhalten und meistens auch eine Körperschemastörung. Das Selbstwertgefühl der Betroffenen ist in sehr hohem Maße abhängig von Gewicht und Figur.
Epidemiologie
Die Inzidenz der Essstörungen dürfte sich in den letzten 15 Jahren nicht wesentlich verändert haben. Allerdings beträgt die Prävalenz von AN und BN zusammen in der Risikogruppe der Frauen zwischen 14 und 35 Jahren 5 %.
Ätiologie
Zur Entstehung von Essstörungen tragen genetische, neurobiologische, psychosoziale Faktoren sowie akute und chronische Belastungen bei.
Verlauf
Die AN weist eine 10-Jahres-Letalität von 5,6 % auf. Damit ist sie 12-mal so hoch wie die Sterberate in der Normalbevölkerung in dieser Altersklasse. Im Verlauf der AN kommt es bei ca. 50 % zu einer Remission, bei ca. 25 % zu einer Besserung und bei 10–20 % zu einer Chronifizierung. Zu Übergängen in eine BN kommt es bei etwa einem Drittel der Fälle.
Verlauf der BN: Etwa 50 % haben nach mehr als 5 Jahren keine Symptome mehr, ca. 20 % leiden weiterhin am Vollbild der BN, ca. 30 % erleben einen Wechsel zwischen symptomfreien Phasen, Rückfällen und subklinischer Ausprägung. Übergänge in eine AN sind äußerst selten.
Komorbidität
Häufige komorbide Störungen sind affektive Störungen, Angststörungen, bestimmte Persönlichkeitsstörungen sowie Substanzmissbrauch und -abhängigkeit.
Organische Komplikationen
Die somatischen Komplikationen bei der AN sind größtenteils mit dem Starvationssyndrom verknüpft und in der Regel bei Gewichtsnormalisierung reversibel. Bei Betroffenen mit bulimischer Symptomatik muss vor allem auf den Kaliumwert geachtet werden.
Initialdiagnostik
Das wichtigste Instrument zur Diagnosestellung ist die Exploration.
Therapiemöglichkeiten
Psychotherapie gilt als Therapie der ersten Wahl. Multimodale Ansätze unter Berücksichtigung störungsspezifischer symptomorientierter Behandlungsaspekte sind zu empfehlen. SSRI sind bei BN hilfreich. Im Fokus der Therapie sollte die Überbewertung von Figur und Gewicht, deren Wichtigkeit für den Selbstwert und das Essverhalten stehen.
