Aktuelle sporttherapeutische Aspekte zur Behandlung der Inkontinenz nach Prostatektomie – Evaluation eines Übungsprogramms

Nach radikaler Prostatektomie nimmt die Behandlung der durch die Operation ausgelösten Harninkontinenz einen wesentlichen Stellenwert in der komplexen urologischen Rehabilitation ein. Die Sporttherapie hat sich in diesem Feld – ganz im Gegensatz zu anderen Indikationen – mit ihren Konzepten bislang noch nicht etabliert. Der vorliegende Artikel zeigt zum einen auf, mit welchen Inhalten die Sporttherapie die urologische Rehabilitation inhaltlich unterstützen kann. Dabei wird zunächst auf ein zielgerichtetes Training der Schließmuskulatur fokussiert, das zunehmend in komplexere Trainingskonzeptionen integriert wird. Ein solches Training muss sich vom bekannteren Training der Beckenbodenmuskulatur bei Frauen deutlich unterscheiden. Zum anderen wird eine Pilotstudie vorgestellt, die die neuromuskuläre Ansteuerung der Beckenbodenmuskulatur untersucht. In einem 8-Kanal-Setup wird die physiologische neuromuskuläre Aktivität sowohl in alltäglichen Haltungen und Bewegungen als auch in Übungssituationen evaluiert. Ziel ist es, geeignete Übungen herauszuarbeiten, die im Rahmen eines Rehakonzepts eingesetzt werden können. Die Ergebnisse belegen eine enorme Einbindung der Schließmuskulatur in sämtliche Haltungen und Bewegungen sowie bei unterschiedlichen Übungen.

Abstract

The treatment of the subsequent incontinence after radical prostatectomy plays an important role in the complex urological rehabilitation. The contribution of sport therapy in this field, compared to other indications, is yet to be established. The article therefore seeks to close this gap by highlighting the subject matter of sport therapy and the role it can play in the sphere of urological rehabilitation. Emphasis is put on target oriented training of the pelvic floor muscle contraction that has increasingly become one of the integral parts in the complex training concepts. However, such training must be differentiated from the famous pelvic floor training for women. Additionally the article introduces a pilot study that examines the neuro-muscular regulation of the pelvic floor muscle. In an eight channel setup, the physiological muscular activities, in both daily posture and movement as well as during exercising, are evaluated. The objective is to formulate a suitable exercise that is utilisable in the rehabilitation concept. According to the findings, pelvic floor muscle contraction should be included in all posture and movement as well as other exercises.
