Zusammenfassung

Trikuspidalklappeninsuffizienz (TI) ist eine häufige Begleitdiagnose bei Patienten mit einem dominierend linkskardialen Vitium oder auch einer myokardialen Dysfunktion. Daneben können Patienten mit einer Reihe von anderen Grunderkrankungen, wie etwa infektiöser Endokarditis oder auch schrittmacherpflichtiger Herzrhythmusstörung, von einer isolierten TI betroffen sein. Bei schwerer Ausprägung der TI stellt eine Operation in vielen Fällen den heute praktizierten therapeutischen Standard dar, wenn auch die aktuelle Datenlage sehr lückenhaft ist und die Empfehlungen der Fachgesellschaften einen niedrigen Evidenzlevel aufweisen. Diese Arbeit beschreibt die heute verfügbaren chirurgischen Optionen zur Therapie der primären und sekundären TI mit Berücksichtigung der komplexen diagnostischen und therapeutischen Konstellation im Falle der letztgenannten Form.

Abstract

Tricuspid valve regurgitation (TR) commonly accompanies left-sided heart disease and also other conditions associated with myocardial dysfunction. Further, in a number of other diseases, e.g. infective endocarditis, rhythm disorders necessitating the use of a permanent pacemaker etc., patients may present with isolated TR. Surgery represents the standard of care in many cases with severe TR. However, solid clinical data for most forms of tricuspid valve surgery are yet scarce. Likewise current guidelines concerning tricuspid valve surgery exclusively demonstrate low level of evidence. This work describes currently available techniques of surgical treatment of primary and secondary TR, considering the complexity of the diagnostic and the therapeutic path for the latter form.

  • Rekonstruktive Verfahren durch Anuloplastie und ggf. Segelrekonstruktion oder Chordaplastik sollten vor dem Hintergrund der prothesenassoziierten Komplikationen in der Trikuspidalklappenposition gegenüber einem Klappenersatz wo immer möglich den Vorrang erhalten.

  • Bei primärer Trikuspidalklappeninsuffizienz (TI) im Erwachsenenalter können lokal begrenzte strukturelle Defekte an den Trikuspidalklappensegeln und subvalvulären Elementen als Folge von entzündlichen Prozessen (infektiöse Endokarditis) oder auch nach mechanischer Schädigung durch Katheter oder bei Adhäsion mit chronischen Implantaten (z. B. Schrittmachersonden) in der Regel mit einer Kombination aus Segelplastik und Anuloplastie behoben werden.

  • Eine sekundäre (funktionelle) TI bei TK-Ringdilatation (z. B. infolge langjährigem Vorhofflimmern) oder bei RV-Dilatation und pulmonaler Hypertonie lässt sich meistens durch Anuloplastie therapieren, allerdings könnte bei massiver RV-Dilatation ein Klappenersatz hinsichtlich Vorhersagbarkeit des funktionellen Ergebnisses zu bevorzugen sein. Hierbei stellt das erhöhte Risiko eines perioperativen Rechtsherzversagens insbesondere bei Vorliegen einer schweren pulmonalen Hypertonie eine diagnostische und therapeutische Herausforderung dar.
