Komplexe Behandlungsprinzipien in der orthopädischen Rheumatologie

Aufgrund der Tatsache, dass der Krankheitsverlauf durch die seit der Jahrtausendwende eingesetzten Biologika verlangsamt wird, sind die konservativen und gelenkerhaltenden operativen Therapien von essenzieller Bedeutung, bevor der Ersatz oder die Versteifung von Gelenken notwendig werden. Der Beitrag stellt die allgemeinen Prinzipien der ganzheitlichen Behandlungen in der Orthopädischen Rheumatologie dar.
Kernaussagen

  • Die Therapie der entzündlich-rheumatischen Erkrankungen wurde durch die Einführung der Biologika um das Jahr 2000 deutlich verbessert, da durch diese moderne immunsuppressive Therapie die Gelenkzerstörung deutlich langsamer voranschreitet.

  • Durch das langsamere Voranschreiten der Erkrankung gewinnen die konservativen Behandlungsmethoden an Bedeutung.

  • Die entzündlich-rheumatischen Erkrankungen sind Systemerkrankungen, und demzufolge ist eine ganzheitliche Therapie erforderlich.

  • Die allgemeinen Prinzipien einer solchen ganzheitlichen Therapie im Bereich der Rheumatologie lauten:

    • Ganzkörperstatus vor jeder Therapie erheben.

    • Korrekturen von Extremitätenfehlstellungen von proximal nach distal.

    • Synovialektomien immer radikal.

    • Ganzheitlicher Therapieplan bei Systemerkrankungen.

    • Komorbiditäten beachten.

    • Die konservative Therapie rheumatischer Erkrankungen ist anspruchsvoll.

  • Die Spezialisierung in der Orthopädie und Unfallchirurgie ist so weit fortgeschritten, dass sie als unumkehrbar gelten darf. Gerade deshalb ist es sehr wichtig, dass die Spezialisten, zumindest in ihrer Körperregion, auch die Behandlung der entzündlich-rheumatischen Veränderungen sach- und fachgerecht durchführen und sich das dafür notwendige Wissen aneignen.

  • Dies bedeutet auch, dass der Orthopäde die medikamentöse Therapie einleitet und überwacht, wenn eine internistisch-rheumatologische Betreuung nicht möglich ist.
