ZUSAMMENFASSUNG
Für Jugendliche mit Mukoviszidose ist die Transition von der Kinder- und Jugendmedizin zur erwachsenenmedizinischen Betreuung oft ein sehr großer Schritt. Nicht selten treten dabei Probleme auf, die sich ungünstig auf ihre Therapie und ihren Gesundheitszustand auswirken. In diesem Beitrag werden Ergebnisse einer explorativen Studie vorgestellt, in der medizinische und therapeutische Fachkräfte zu ihren Erfahrungen mit der Transition von jungen Mukoviszidosepatienten befragt wurden. Als hinderlich für die Transition wurden insbesondere unterschiedliche Behandlungsansätze in Jugend- und Erwachsenenmedizin beschrieben, ferner Ängste und Vorbehalte, ein schlechter Gesundheitszustand des Patienten, Compliance-Probleme sowie eine unzureichende Kooperation der Behandler aufgrund limitierter Kapazitäten. Als Gelingensfaktoren wurde eine frühzeitige Vorbereitung der Transition genannt, ferner ein klarer Schnitt bei Volljährigkeit, kontinuierliche Ansprechpartner, multidisziplinäre Kooperation, psychosoziale Beratung zu Themen des Erwachsenwerdens sowie ein systemischer, ressourcenorientierter Ansatz.
ABSTRACT
For adolescents with cystic fibrosis, the transition from pediatric and adolescent medicine to adult medical care is often a big step. It is not uncommon for problems to arise during this process that have an unfavorable impact on their therapy and health status. This paper presents results of an exploratory study in which medical and therapeutic professionals were interviewed about their experiences with the transition of young cystic fibrosis patients. In particular, different treatment approaches in adolescent and adult medicine were described as barriers to transition, as well as fears and reservations, poor health status of the patient, compliance problems, and insufficient cooperation among treatment providers due to limited capacity. Early preparation of the transition, a clear cut at the age of majority, continuous contact persons, multidisciplinary cooperation, psychosocial counseling on issues of growing up, and a systemic, resource-oriented approach were mentioned as factors for success.
