Die akute Lungenarterienembolie (LAE) stellt weiterhin die dritthäufigste kardiovaskuläre Todesursache dar und weist eine weltweit steigende Inzidenz auf. Sich kontinuierlich neu entwickelnde, vornehmlich katheterbasierte Therapieoptionen gepaart mit einem Mangel an Daten aus klinischen randomisierten Studien führen aktuell zu einer Herausforderung für alle an der Therapieentscheidung der akuten LAE beteiligten, klinisch tätigen Ärzte, insbesondere bezogen auf die „Intermediär-hoch“-Risikogruppe.
Um diesem Umstand Rechnung zu tragen und mit dem Ziel, durch individuellere Therapieentscheidungen die Letalität der LAE-Patienten zu senken, wurden in den letzten Jahren vor allem in den USA interdisziplinäre „Pulmonary Embolism Response Teams“ (PERT) gegründet. In diesem Artikel wird neben einer Zusammenfassung der in den aktuellen ESC-Leitlinien von 2019 aufgeführten Risikostratifizierung und Therapieempfehlungen auch auf den Einfluss der PERT auf das Management und das Outcome der akuten LAE eingegangen und offene Fragen werden adressiert.
Abstract
Pulmonary embolism is a common cardiovascular disease and still the third most common cause of cardiovascular mortality with an in-hospital mortality of almost 15%. The quick identification of vitally threatened patients and, in particular, rapid treatment of the patients still represents a challenge. The „Pulmonary Embolism Response Team“ (PERT) can be helpful in making this therapy decision and is an interdisciplinary team adapted to local resources for faster and more coordinated treatment. New catheter-based procedures (ultrasound-guided catheter lysis or mechanical thrombus aspiration) without the need for systemic lysis could revolutionize the treatment of pulmonary artery embolism.

Die Lungenarterienembolie (LAE) ist die dritthäufigste kardiovaskuläre Todesursache hinter Myokardinfarkt und Schlaganfall.

Die Intrahospitalletalität der LAE liegt bei fast 15%.

Patienten mit LAE und intermediär-hohem Risiko (bildgebende Zeichen der Rechtsherzbelastung und erhöhtes Troponin) sollten frühzeitig identifiziert werden, da sie von einer interventionellen, katheterbasierten Therapie (ultraschallgestützte Katheterlyse oder mechanische Thrombusaspiration) ohne Notwendigkeit einer systemischen Lyse zu profitieren scheinen.

Das „Pulmonary Embolism Response Team“ (PERT) ist ein an die lokalen Ressourcen angepasstes, interdisziplinäres Team zur schnelleren und koordinierteren Behandlung der akuten LAE und sollte zumindest aus interventionellen Kardiologen, Intensivmedizinern und Radiologen bestehen.
