Tabakentwöhnung – die bisher wenig genutzte Chance im ärztlichen Alltag
Tobacco cessation – the underused potential in clinical routineRauchen verursacht jährlich in Deutschland etwa 120 000 vorzeitige Todesfälle und ist damit der größte vermeidbare Risikofaktor für die Entstehung und den Verlauf vieler Erkrankungen und der COVID-19-Infektion. Es verwundert nicht, dass sich etwa drei Viertel aller Raucher wünschen, niemals angefangen zu haben. In Deutschland versuchen etwa 20% der Raucher einen Aufhörversuch pro Jahr, allerdings nur zu 1% mit evidenzbasierten Methoden oder ärztlicher Hilfe.
Abstract
Tobacco control, psychosocial and medical assistance regarding tobacco cessation is still a hidden potential within the German health care system. So far doctors rarely talk to their patients about their smoking status and physical and psychological benefits of quitting.
This paper focusses on recommended current diagnostic and treatment standards, as well as evidence-based methods to address the topic on how to stop smoking and its association with certain diseases such as COPD, lung cancer and COVID-19 infection. The role of e-cigarettes as a cessation tool and its health related risks are critically examined. Consequences and advice how to implement smoking cessation procedures into daily practice are presented.

Jeder Raucher soll eine ärztliche Empfehlung zum Rauchstopp erhalten.

Die Behandlung abhängiger Raucher ist durch eine Kombination aus medikamentöser Unterstützung in Verbindung mit Beratungs- und psychotherapeutischen Maßnahmen zu erreichen.

Eine Tabakentwöhnung mit medikamentöser Unterstützung soll unter stationären Bedingungen begonnen und dann in ambulanten Strukturen weitergeführt werden.

E-Zigaretten werden zur Tabakentwöhnung nicht empfohlen.

Bei COVID-19 ist für Tabakraucher von einem erhöhten Risiko schwerwiegender Krankheitsverläufe und einer erhöhten Krankenhaussterblichkeit auszugehen.

Bei COPD sollte das Tabakrauchen nicht nur reduziert, sondern komplett beendet werden.

In aktuellen Leitlinien wird nachdrücklich empfohlen, dass rauchenden Patienten mit Lungenkarzinom eine Tabakentwöhnung mit psychosozialer und medikamentöser Unterstützung angeboten wird.
