Cinacalcet – ein allosterischer Verstärker am Ca²⁺-Rezeptor

Zusammenfassung

Cinacalcet (Handelsname: Mimpara^®) ist ein allosterischer Verstärker der Wirkung von Ca²⁺-Ionen am Ca²⁺-Rezeptor der Nebenschilddrüsen, einem G-Protein-gekoppeltem Rezeptor. Infolgedessen sinkt die Parathormon-Inkretion und der Ca²⁺-Serumspiegel fällt. Cinacalcet wird peroral angewandt und ist zugelassen zur Behandlung des sekundären Hyperparathyreoidismus bei dialysepflichtigen Patienten mit terminaler Niereninsuffizienz und zur Verminderung der Hypercalcämie bei Patienten mit Nebenschilddrüsenkarzinom. Kontrolliert wird die Therapie durch Messung der Blutspiegel von Ca²⁺ und Parathormon. In klinischen Studien konnte unter chronischer Anwendung von Cinacalcet eine dauerhafte Verminderung des Parathormon-Spiegels gezeigt werden. Ergebnisse aus Langzeitstudien zu klinisch wichtigen Endpunkten, beispielweise Erhalt der Knochenmasse und Frakturschutz, stehen noch aus.

Summary

Cinacalcet (trade name: Mimpara^®) enhances allosterically the action of Ca²⁺-ions at the parathyroid gland Ca²⁺-receptor which belongs to the superfamily of G protein-coupled receptors. As a consequence blood levels of Ca²⁺ and parathyroid hormone decline. Cinacalcet is orally administered and approved for a) the treatment of secondary hyperparathyroidism in patients with end stage renal disease receiving hemodialysis and b) to lower hypercalcemia in patients with parathyroid carcinoma. Therapeutic monitoring includes measurement of blood levels of Ca²⁺ and parathyroid hormone. The stable suppression of parathyroid hormone levels under chronic treatment was shown in clinical trials. Clinically relevant outcome parameters, such as bone mass and fracture risk, remain to be evaluated.
