Zusammenfassung
Anhand von 27 Fällen werden die Anwendungsmöglichkeiten der perkutanen Nadeltrepanation untersucht und einige Modifizierungen vorgestellt. Es wird auf die Brauchbarkeit dieser einfachen und komplikationsarmen Methode für Vorderhornpunktionen zur Ventrikulographie, Dauerdruckmessung nach Fixierung der Punktionsnadel mit Gips, Druckentlastung oder Antibiotika-Applikation und in erweiterter Indikation auch für die Punktion von Zysten und Biopsien von Tumoren hingewiesen
Einleitung
Nachdem bereits 1965 Burton und Blacker (7) die Verwendung eines Hand-Drillbohrers zur notfallmäßigen Schädeltrepanation empfahlen, beschrieben Rifkinson et al. (1) 1973 die Punktion des Vorderhorns mittels perkutaner Nadeltrepanation. Diese erwies sich nach den Erfahrungen von Vadura et al. an weiteren 15 Patienten als brauchbare Methode. Letzterer empfiehlt zur schnelleren Trepanation den Einsatz eines batteriebetriebenen Hand-Drillbohrers. Eine zusätzliche Verwendung ergab sich bei der Behandlung chronischer Subduralhämatome (5). Vorteile der als komplikationslos angegebenen Methode sind neben der geringen Belastung für den Patienten rasche, einfache und sichere Handhabung. Ihre Anwendung beschränkt sich bisher auf Ventrikeldarstellungen mit Kontrastmittel und Isotopen, Ventrikeldruckmessungen und -entlastungen sowie intrathekale Applikation von Antibiotika.
Wir wandten die perkutane Nadeltrepanation 1976/77 technisch modifiziert und unter erweiterter Indikation bei 27 Patienten mit verschiedenen intrakraniellen raumfordernden Prozessen an.
Summary
Based on experiences in 27 cases the possibilities of the use of percutaneous twist-drill needle exploration are investigated and a few modifications suggested. The usefullness of this simple and complication-free technique is emphasised. It can be used for puncture of the anterior horn, for ventriculography, continuous pressure measurements after fixation of the puncture needle with plaster, relief of pressure or injection of antibiotics and, as further applications, also for the puncture of cysts and the biopsy of tumours.
After Burton and Blacker in 1965 (7) recommended the use of a hand-drill for emergency skull puncture, Rifkinson et al. (1) in 1973 described the use of twist-drill puncture of the anterior horn of the ventricle. After the experiences of Vadura et al. in a further 15 patients this was shown to be a useful technique. More recently the introduction of a batterydriven twist drill has been recommended for more rapid perforation of the skull. A supplementary use has been for the treatment of chronic subdural haematomas (5). Apart from the minimal stress for the patient the advantages of the method, which as described is complication-free, are its rapid, simple and certain application. Its use has so far been confined to demonstrating the ventricles with contrast medium or isotopes, measurement or relief of ventricular pressure and also the injection of antiobiotics directly into the CSF pathways.
During 1976/77 we used percutaneous twist-drill puncture, with technical modifications and with wider indications, in 27 patients with various intracranial space-occupying lesions.
