Summary
The diagnosis of amyotrophic lateral sclerosis was originally considered in an atypical case of cervical disc protrusion with spastic paraparesis and muscle atrophy without root symptoms. The antecedent history and subsequent clinical course however pointed finally, even prior to several clinical examinations, to the correct diagnosis of unilateral (hemi) cord compression due to a lateral disc protrusion.
Zusammenfassung
Ein uncharakteristisches Bild von cervicalem Discusprolaps mit spastischer Parese und Muskelatrophie ohne nennenswerte radiculäre Symptome wurde auswärts als myatrophische Lateralsklerose (MALS) angesehen. Die treffende Anamnese bzw. Auswertung des Verlaufsgeschehens war für uns bereits vor verschiedenen Untersuchungen zur entscheidenden Diagnosestellung einer halbseitigen Halsmarkkompression durch einen komprimierenden Discusprolaps richtungsweisend.
Résumé
Un tableau peu caractéristique d'une hernie discale cervicale avec atrophie musculaire spastique sans symptomatologie radiculaire pertinente avait été cosidéré comme dû à une sclérose latérale amyotrophique. Une anamnese bien interprétée, nous dirigea, avant tout autre examen, vers le diagnostic d'une compression médullaire latérale par hernie discale.
