Summary
10 cases of gunshot injuries of the spinal cord and cauda equina are reported. Immediate operation with simultaneous administration of dexamethasone, the use of modified hypothermia and extensive dura plasty is recommended. In 4 patients a suspected total severance of the cord was verified by operation. Any effect on neurological losses and rehabilitation could not be demonstrated. 2 patients died (one of sepsis due to intestinal tears, one of a pulmonary complication with a high transverse lesion). At least 2 patients showed striking improvement which may be related to immediate operation.
Zusammenfassung
Es wird über 10 Fälle von Schußverletzungen der Medulla und der Cauda equina berichtet. Sofortige Operation mit gleichzeitiger Dexamethason-Behandlung, die Anwendung einer modifizierten Hypothermie und eine ausgedehnte Duraplastik werden empfohlen. Bei 4 Patienten wurde die vermutete totale Durchtrennung der Medulla durch die Operation bestätigt. Ein Effekt auf das neurologische Defizit und die Rehabilitation konnte hier nicht gezeigt werden. 2 Patienten starben (einer an Sepsis infolge intestinaler Verletzungen, einer an einer Lungenkomplikation bei hohem Querschnitt). Mindestens 2 Patienten zeigten eine erhebliche Besserung, die im Zusammenhang mit der sofortigen Operation zu stehen scheint.
Résumé
On rapporte 10 cas de blessures par coups de feu de la moelle et de la queue de cheval. On recommande l'intervention immédiate avec administration simultanée de dexamethasone, application d'une hypothermie modifiée et plastie étendue de la duremère. La section totale de la moelle supposée chez 4 patients a été vérifiée à l'intervention. Il n'y eut aucun effet sur le déficits neurologiques et sur la réhabilitation. 2 malades sont morts (l'un par infection partie d'une déchirure intestinale, l'autre de complications pulmonaires liées à une blessure transverse haute). Au moins 2 sujets montrèrent une amélioration nette à mettre sur le compte de la précocité de l'intervention.
