Summary
A study was carried out on 110 cases of craniopharyngioma, admitted to the Clinic of Neurosurgery of Bucharest, between 1936 and 1970. Clinical statistical analysis showed that the condition is characteristic of children and younger age groups, 95 cases belonging to the first three decades of life. The onset and subsequent symptomatology was caused by the compression produced by the tumour on the neighbouring autonomic neuro-endocrine formations, the signs varying in terms of the volume of the tumour and age of the patients. The diagnosis is not always easy and in children may be confused with tumours of the posterior cranial fossa, and in the adult with predominant psychical disturbances, with tumours of the frontal lobe. The treatment was surgical and the results close to those reported in literature, noting a net improvement in the last ten years when total removal (15 cases) was possible in the cases admitted at an earlier date. The application of intensive therapy and hormonal substitutive drugs evidently contributed to lowering the immediate mortality rate. Radioresistance of craniopharyngioma was found, although several encouraging results have been reported, without however having been checked with time.
Zusammenfassung
Es wurden HO Fälle von Craniopharyngeom untersucht, die zwischen 1936 und 1970 an der Neurochirurgischen Klinik von Bukarest stationär aufgenommen wurden. Die klinisch-statistische Analyse zeigte, daß diese Erkrankung charakteristisch für das Kindes- und jugendliche Alter ist; 95 Fälle gehörten in die ersten drei Lebensdekaden. Der Beginn und die weitere Symptomatologie wurden durch den Tumordruck auf die benachbarten autonomen neuroendokrinen Formationen hervorgerufen, die Symptome waren abhängig von der Größe des Tumors und dem Alter des Patienten. Die Diagnose ist nicht immer einfach, bei Kindern können die Erscheinungen mit Tumoren der hinteren Schädelgrube verwechselt werden und bei Erwachsenen, sofern psychische Erscheinungen überwiegen, mit Tumoren des Frontallappens. Die Behandlung erfolgte auf chirurgischem Wege, und die Ergebnisse waren den in der Literatur berichteten ähnlich, wobei eine Verbesserung der Ergebnisse in den letzten 10 Jahren bei den Fällen mit totaler Entfernung (15 Fälle) möglich war, wenn sie frühzeitig in Behandlung kamen. Der Einsatz der Intensivtherapie und von hormonalen substituierenden Medikamenten trug offensichtlich zu einer Verminderung der Frühmortalität bei. Beim Craniopharyngeom war eine Röntgenunempfindlichkeit festzustellen, obgleich über einige ermutigende Ergebnisse berichtet wurde, ohne daß jedoch längere Verläufe bekannt sind.

Etude de 110 cas de craniophryngiomes admis à la clinique de neurochirurgie de Bucarest entre 1936 et 1970. Un relevé statistique montre que le tableau clinique est caractéristique pour l'enfant et l'adolescent, 95 % des cas appartenant aux trois premières décennies de la vie. Le début et l'évolution de la symptomatologie étaient déterminés par la compression des structures neuro-endocriniennes voisines par la tumeur et variaient suivant le volume de cette dernière et l'âge de son porteur. Le diagnostic n'est pas toujours facile et peut aiguiller vers une tumeur de la fosse postérieure chez l'enfant, ou une tumeur frontale chez l'adulte si les troubles psychiques sont prédominants. Le traitement était chirurgical, et les résultats, concordants avec ceux de la littérature, comportaient une nette amélioration au cours des dix dernières années du fait de la possibilité d'une excision totale (15 cas) et d'une hospitalisation plus précoce. L'application d'une thérapeutique intensive et de drogues hormonales substitutives a contribué manifestement à diminuer le taux de mortalité immédiate. Une résistance aux rayons X a été constatée, encore que plusieurs résultats favorables aient été observés, sans toutefois avoir été contrôlés dans la suite de l'évolution.
