Abstract
Report about a five-month-old male infant who developed an extreme muscle rigidity immediately after administration of succinylcholine and introduction of general anesthesia with halothane and nitrous oxide/oxygen. Further typical symptoms of the so-called “malignant hyperpyrexia” occurred: elevation of serum-CPK up to 3000 mU/ml, hyperpotassemia, coagulopathia, tachycardia, tachypnoe and cyanosis in spite of forced artificial respiration and mixed respiratory and metabolic acidosis (pH 7.04). Exitus letalis 13 hours after introduction of general anesthesia. The lack of fulminantly developing excessive hyperthermia as a leading symptom, after which the syndrome was named “malignant hyperpyrexia” is explained partially by particularities of heat regulation in early infancy and special exogenic circumstances.
Light- and electron microscopical investigations of nine different skeletal muscles showed striking changes in all of them: 1. Results of an extreme mechanical stress , such as fragmentations and contraction figures, obviously caused by the general rigidity; 2. acute rhabdomyolysis with floccular or hydropic-vacuolizing degeneration of the severely damaged muscle fibres or their fragments; these changes are mainly regarded as the expression of an energetic-dynamic insufficiency of skeletal muscle. Because malignant hyperpyrexia as a symptom is evidently due to a disorder of skeletal muscle metabolism and moreover obviously not obligatory in infancy, it is suggested to call these incidents “acute anesthesia-induced myopathy” to definitely include cases as the present one, because they surely belong to them from the etio-pathological viewpoint.
Zusammenfassung
Bericht über einen 5 Monate alt gewordenen Knaben, bei dem kurz nach Einleitung einer Halothan-Lachgasnarkose und Succinylcholingabe eine extreme Muskelstarre auftrat und sich die weiteren, für die „maligne Hyperpyrexie” typischen Symptome einstellten: Erhöhung der Serum-CPK (3000 i. E./l), Hyperkaliämie, Koagulopathie, Tachykardie, Tachypnoe und Zyanose trotz forcierter Beatmung, respiratorisch-metabolische Azidose. Tod 13 Stunden nach Einleitung der Narkose. Das Ausbleiben der sonst während dieser Zwischenfälle bei älteren Individuen auftretenden, bislang namengebenden Hyperthermie wird hier teils auf altersbedingte Besonderheiten der Regulation des Wärmehaushaltes, teils auf exogene Faktoren zurückgeführt. Mikroskopisch fanden sich
die Folgen einer extremen mechanischen Überbeanspruchung in Form von Fragmentationen und Kontraktionsfiguren, offenbar bedingt durch die Muskelstarre;
eine akute Rhabdomyolyse , bei der die schwer geschädigten Fasern einer scholligen oder hydropis&-vakuolären Degeneration anheimfallen; diese wird im Wesentlichen als Ausdruck einer energetisch-dynamischen Insuffizienz des Muskels gewertet.
Da die Hyperpyrexie offenbar Folge des durch die Narkose gestörten Muskelmetabolismus und außerdem, zumindest im Säuglingsalter, nicht obligat zu sein scheint, wird für diese Gruppe von Narkosezwischenfällen die Bezeichnung „akute, anästhesie-induzierte Myopathie” vorgeschlagen.
