Microanalysis with ¹⁴C-Dansyl Chloride of Amino Acids and Amines in the Cerebrospinal Fluid of Patients with Phenylketonuria – II. Determination of 5-hydroxyindole derivatives after loading with L-phenylalanine

Abstract

Twenty-seven children with classical phenylketonuria (PKU) 1—7œ years of age being on a constant diet with 17—35 mg phenylalanine/kg/die, were loaded with 0.1 g/kg L-phenylalanine. Amines and amino acids were determined in the cerebrospinal fluid (CSF). After reaction with ¹⁴C-dansyl-chloride and two dimensional microchromatography the indole derivatives were analysed and the results compared with 5 control children 3 mo.—11œ yrs. of age. In 20 PKU children CSF-phenylalanine was greatly increased and 5-hydroxytryptophan could be detected. A significant positive correlation exists between the concentrations of phenylalanine and 5-hydroxytryptophan in the CSF. In the 7 cases with relatively low CSF-phenylalanine concentration and in the 5 control children 5-hydroxytryptophan was not detectable in the CSF. Serotonin could be detected in 22 of the PKU—CSF samples but not in the “controls”. The CSF concentration of 5-hydroxyindole acetic acid was below the limit of detectability in both PKU and “controls”. It is concluded from these results that an increase of phenylalanine concentration does not lead to a significant inhibition of tryptophan-5-hydroxylase or 5-hydroxytryptophan decarboxylase in the central nervous system.

Zusammenfassung

Siebenundzwanzig Kinder im Alter von 1–7œ Jahren mit unterschiedlichem Intelligenzquotient, die auf eine Diät von 17-35 mg Phenylalanin/kg/die eingestellt waren, wurden nüchtern mit 0.1 g/kg Körpergewicht L-Phenylalanin belastet. Nach Umsetzen mit ¹⁴C-Dansylchlorid und zweidimensionaler Mikrochromatographie wurden die Indolderivate 5-Hydroxytryptophan, Serotonin und 5-Hydroxyindol im Liquor analysiert und die Befunde mit 5 normal ernährten Kontrollkindern im Alter von 3 Mo. bis 11œ Jahren verglichen. Bei 20 PKU-Kindern war Phenylalanin im Liquor stark erhöht, und gleichzeitig konnte 5-Hydroxytryptophan nachgewiesen werden. Zwischen der Phenylalanin- und 5-Hydroxytryptophankonzentration im Liquor besteht eine signifikante positive Korrelation. Bei den 7 Fällen mit niedriger Phenylalaninkonzentration und den 5 Vergleichsfällen war 5-Hydroxytryptophan nicht nachweisbar. In 22 der PKU Liquores, aber nicht in den Vergleichsfällen konnte Serotonin nachgewiesen werden. 5-Hydroxyindolessigsaure war sowohl bei den PKU-Kindern als auch bei den Kontrollen unterhalb der Grenze der Nachweisbarkeit. Aus diesen Befunden ist zu folgern, daß eine Erhöhung des Phenylalaninspiegels nicht zu einer nennenswerten Hemmung der Tryptophan-5-Hydroxylase oder der 5-Hydroxytryptophandecarboxylase im Zentralnervensystem führt.
