Zusammenfassung
Ein traumatischer Sehnervenschaden stellt eine schwere Komplikation eines Kopf- bzw. Gesichtstraumas dar. Die Diagnose erfolgt mittels Analyse des Pupillenverhaltens nach Lichtreiz. Eine Untersuchung des Augenhintergrundes und die Computertomografie der Orbita geben Aufschluss über den Schadensort und ob eine Fraktur des Optikuskanals vorliegt. Therapieoptionen sind operative Dekompression und hoch dosierte Steroide. Bei keinem Therapieansatz gibt es hinreichende Belege, dass der Spontanverlauf günstig beeinflusst werden kann. Spontane Besserung ist durchaus möglich. Aufgrund von Einzelerfahrungen werden direkte Kompressionen des Sehnerven durch Knochenfragmente oder Blutungen in die Optikusscheide in aller Regel operativ behandelt. In den meisten Kliniken werden in allen anderen Fällen hoch dosierte Steroide gegeben, ohne dass es aber einen Wirkungsnachweis gibt.
Abstract
Traumatic optic neuropathy is a severe complication of a head or face trauma. Diagnosis is established by analyzing pupillary responses to light. Fundus examination and orbital CT scan help to localize the site of the lesion and may show a fracture involving the optic canal. Surgical decompression or megadose steroids are therapeutic options. No therapeutic approach has proven with sufficient evidence to be effective. Spontaneous improvement is definitely possible. Based on experiences in single cases direct compression of the optic nerve by bone fragments or optic nerve sheath haemorrhage are as a rule treated surgically. In all other cases usually high dose steroids are given without proven effectiveness.
